Process for Admission after being Accepted to Abilities Inc
- TB test and physical screening must be completed prior to admission
- Physical Examination Form
- Permission to administer medications to be signed by prescribing physician
- Tb form to be signed by nurse or reading physician
2. Physician order of medications must be sent to our pharmacy

3. Admission Paperwork and Agreement (2) packet must be completed and mailed or faxed back to our Corp office

4. For those admitted to Supported Living Program
- a Signed lease must be completed with SV Rentals (Management for MisEnid LLC and Rod Lee)
- Current Rent Per month is 550 per Month for shared 2 bedroom apartment (utilities Included)
- Current Rent per month is 600 per Month for 1 bedroom apartment (utilities included)
5. Move in Arrangements
- Abilities does not move major belonging or clean out prior places that member lived in
- Abilities will transport the member and what can fit in a mini van
- clothing, TV, recliner are acceptable
- Larger Furniture must be moved by family or furniture company. The Case manager or guardian will coordinate these needs
